Bring the following documents and information when registering a new student:
- Certified birth certificate
- Proof of residence-utility bill or lease agreement
- Florida immunization form (blue sheet from health dept, or doctor)
- Physical (can't be over one year old and must include the doctor's name and address if from out of state)
- Copy of Social Security Card
